Name, Social Security Number, and/or Date of Birth Changes

To change your name, SSN, or date of birth with the University you will need to

log onto RAMweb and
click on the “Change My Name, SSN, or Date of Birth” link under the Records section.
Complete the update form and submit it, along with the required documentation as outlined on the form, to the Registrar’s Office.

The form and documentation can be submitted by email at registrarsoffice@colostate.edu, or in person in Centennial Hall. We are unable to process changes from faxed or mailed documents.
